Statewide Medicines Formulary Policy

Applicable to: This Policy is applicable to all Health Service Providers that provide medicines to the public. It applies to all prescribers to guide appropriate and equitable access to medicines and medicine management governance.

Description: The purpose of the Statewide Medicines Formulary (SMF) Policy (the policy) is to assist the delivery of optimal patient outcomes in an equitable manner through a single list of approved medicines. The rationale for the SMF is underpinned by the Australian National Medicines Policy which aims to improve positive health outcomes focusing on patient's access to, and wise use of, medicines. The SMF will support efficiencies in medicines cost, use and governance, will improve uniformity and transparency, promote the safe and quality use of medicines and provide opportunities for future quality and safety improvement initiatives related to medicines.

All eligible prescribers working at health service providers are required to initiate medicines according to the restrictions and requirements stated by the SMF.
