Patient Information Retention and Disposal Schedule Policy

Applicable to: This policy is part of the Information Management Policy Framework and is applicable to all staff members (employees, contractors, students, volunteers and agency personnel) of the Department, Health Service Provider or a person engaged under a contract for service by the Minister, Department CEO or a Health Service Provider.

Description:The Patient Information Retention and Disposal Schedule Version 4,2014 (Schedule) provides a management tool for identifying and determining the retention and disposal requirements for patient records created or received by health care facilities within WA Health. The Schedule contains valuable information and provides guidance to WA Health with respect to the major categories of patient records. It is therefore an important business tool and assists compliance with legal obligations. The Schedule must be read in conjunction with the Digitisation and disposal of Patient Records Policy and the policy Toolkit. Patient records already sentenced under previously approved retention and disposal schedules which are still in the custody ofWA Health, will be resentenced in accordance with this Schedule.
