Winner: The 'Forget Me Not' Volunteer Team, Royal Perth Bentley Group

The 'Forget Me Not' volunteers are a team of trained members of the community who provide person centred care, emotional support and practical assistance to vulnerable patients on the Aged Care ward at Royal Perth Hospital (RPH). This includes patients with dementia and/or delirium.

As the first of its kind in a metropolitan hospital in WA, this innovative program reflects the organisation’s wider commitment to deliver a better way to care for patients with Cognitive Impairment (CI). The program promotes collaboration between volunteers, staff, patients and their families to create individual, person- centred care profiles.

It is led by a Senior Physiotherapist seconded to a Project Officer role for CI and supported by the multidisciplinary Royal Perth Bentley CI Reference Group.

The 'Forget Me Not' volunteers attended a two day training course delivered by allied health professionals from RPH where they learnt a range of valuable skills to assist patients in various components of their recovery. This included how to deliver person centred communication skills, offer cognitively stimulating activities, taking patients for a walk where safe and prompting with oral replenishment.
