How can you provide feedback?

• Service providers’ survey (external site): provide feedback on the Policy in a professional capacity, including health professionals, health service providers, non-government sectors, local and state government.
• Consumer survey (external site): provide feedback on the Policy from a consumer (someone who uses health services) perspective, including young people in WA aged 10–24, parents, carers and other community members.
• Community Conversations survey (external site): participate in a consumer survey about WA youth health in general.

Each survey should take approximately 10–15 minutes to complete. Your privacy is important to us. Data is stored confidentially and your responses not linked to you in any way.
