About subacute care in WA

Subacute care services aim to improve the quality of life for patients who have a functional impairment by maximising their functional capacity and independence.

Services focus on helping patients through goal-oriented interventions and multidisciplinary management.

Types of subacute care services include:

  • rehabilitation
  • geriatric evaluation and management (GEM)
  • psycho-geriatric care
  • palliative care.
