Mental Health Charts and Clozapine Resources

For facilities that treat mental health patients the following medication charts are mandatory:

• WA Adult Clozapine Initiation and Titration Chart (PDF 255KB)
  This chart facilitates clinical handover and prescription for the safe management of patients initiated or re-titrated on clozapine.
• Agitation and Arousal PRN Chart (PDF 124KB)
• Intramuscular Long-Acting Injection Chart (Depot Antipsychotic) (PDF 89KB)

The charts have been endorsed by:

• WA Therapeutic Advisory Group (WATAG) 
• WA Medication Safety Group (WAMSG)

• EMHS: Clozapine eLearning (accessible to all WA health employees via the WA Health Shared Catalogue on MyLearning)

• WA Clozapine Initiation and Titration Chart Education Resource (PDF 1MB) 
  This PowerPoint has been developed to provide education on features, prompts and alerts in the chart.
• Clozapine Monitoring Form A (PDF 155KB)  |  Clozapine Monitoring Form B (PDF 131KB)
  These forms are designed to document the face to face clinical review and monitoring requirements for consumers on clozapine. These non-mandatory forms can be used after the WA Clozapine Initiation and Titration Chart.

Registry for change

All change requests to the WA Clozapine Initiation and Titration Chart, Agitation and Arousal Chart or Intramuscular Long-Acting Injection Chart (Depot Antipsychotic) should be lodged to the Medicines and Technology Unit.

Recommendations for change must be evidence based, with the primary objective of improving patient safety.
